Waterloo Wellington CCAC (WWCCAC) gets people the care they need to stay well, heal at home and stay safely in their home longer, When home is no longer an option, we help people make the transition to other living arrangements.

Long-term Care Home

Long-term care homes are residential homes that provide ongoing care to patients whose care needs cannot be met  in the community. People may require this level of support  due to advancing age, disability or declining health.

If your care needs becomes too difficult for you and your supports to manage on your own, you can explore care and services available through CCAC. If you need assistance with specific daily activities such as bathing, cooking or decreasing ability to walk safely there may be services that could assist you to stay home with supports in your home or within your community. But if you determine that your needs cannot be met in your home with supports your CCAC Care Coordinator can assist you with planning for other options such as Long Term Care. 

Eligibility and Admission

WWCCAC is your single point of access to information about, and to apply for admission to, Long-Term Care Homes. You cannot apply directly to the Long-Term Care Home. For more information on eligibility and admission please click here.

Long-term Care Home Wait Time Information

Making the decision to move to a long-term care home (LTCH) is a life-changing step for you and your family. It is important that you have all of the information you need to choose the home that is right for you. There are many things to consider, including the level of care you require, location, availability and cost.

Your CCAC Care Coordinator can help you think through your decision. If you are able, we recommend that you and your family visit the homes you are interested in and ask questions before making a final decision.
