Service Description: One-on-one supports for coordinating services, including personal advocacy and filling out forms * interventions for people facing homelessness, critical life changes, abuse, or isolation * referrals to other community support services as necessary

Community navigator will meet with individuals and/or caregivers to provide:
  • health care system navigation
  • assistance in filling out required forms or providing information required to access most appropriate services such as housing, financial support, etc.
  • dealing with elder abuse/hoarding issues
  • referrals to mental health agencies
  • linking to support and services
  • coordination of services with other agencies

The impact of this service is to help people to access the necessities of life and to stabilize their situation by working with the client to develop an action plan, and to offer information regarding programs and services and coordinate services with other agencies
