Service Description: Develops and delivers client-focused community health, support and volunteer services * assists people with health care needs in their own home and community

Nursing services include: 
Visiting nursing
  • normally between 8 am and 12 noon daily, although arrangements can be made for visits at other times
Shift nursing
  • available for blocks of time from four to 24 hours a day for those clients requiring extended care
Specialty nursing
  • nurses who are specialized in palliative care, hospice care, pediatrics, continence management, enterostomal therapy, and wound care
Nursing clinics
Depending on the type of nursing care, patients can be treated at one of three nursing clinics in Burlington, Oakville or Milton * the clinics are for Acclaim Health's Ontario Health atHome Mississauga Halton patients only and are not open to the general public
Personal Support Workers (PSWs) provide home and personal care to those who are elderly * recovering from surgery * illness or injury * living with dementia and other cognitive impairments * adults and children with special needs * facing terminal illness * minimum booking is three hours of care

Services include:
  • bathing
  • skin care
  • dressing
  • grooming
  • feeding
  • exercise
  • general supervision
  • medication supervision
  • caregiver relief
  • laundry
  • meal planning and preparation
Halton Region Lifeline
Partnered with Philips Lifeline Medical Alarm * for anyone who lives alone * is at risk for falls * is recovering from surgery * is managing a medical condition * or for a caregiver for who needs peace of mind * 50% off installation for Acclaim clients

Private care services are also available
  • personal support
  • nursing care
  • foot care (fees vary depending on client's specific foot care needs) * club members attending Adult Day Programs can access foot care on site during their regular day
  • housekeeping
  • companionship
  • grocery shopping assistance
  • meal preparation
  • transportation to appointments
  • safety phone calls check
  • pricing is dependent on the level and duration of care required

Application: Call * no referral required for private-pay services * in-home assessment required for service
