Service Description: Adult day program * social, recreational, and therapeutic activities for people with acquired brain injury in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
  • meals

Access to social work support including:
  • peer support groups
  • caregiver support groups
  • individual counseling

Fees: $5 per day * subsidy available for eligible participants
Application: Contact Ontario Health atHome Waterloo-Wellington, self referrals and referrals from other health care professionals accepted * visit web-site for referral form
Eligibility / Target Population: Older adults and adults with disabilities, who need physical, social, or emotional supports * care coordinators assess the needs of applicants
