Service Description: Short-term caregiver relief program to support older adults who live in their own home but require temporary accommodation due to frailty, depression, social isolation and / or dementia. Services include:
  • 24-hour supervision and monitoring (RPN staff available)
  • assessment and individual program planning
  • supervised therapeutic social and recreational activities
  • care coordination
  • education and counselling
  • community liaison and advocacy
  • adult day program
  • supper club

Fees: $44.38 per night
Application: Call for eligibility inquiries and consultation * referrals through Ontario Health atHome Waterloo-Wellington, family members, friends, neighbours, health and social service agencies, or medical practitioners
Eligibility / Target Population: Eligibility criteria determined by Ontario Health atHome Waterloo-Wellington
Eligibility includes: medically stable * able to tolerate 14 days of isolation * willingness to consent and complete COVID 19 surveillance, testing upon admission to the program and as required/ directed by the Wellington Dufferin Guelph Public Health * must be smoke-free and substance-free * up to 1 person may provide physical assistance for activities of daily living
