Service Description:

Offering support and places to live that address the needs of people with developmental disabilities * independent facilitators provide resources to support families develop a personal creative housing plan
Services include:- relationship building with potential housemates
- searching for an affordable and accessible place to live
- negotiating living arrangements with family, landlords and potential housemates
- forming a network of unpaid supports
- mapping community-based resources in your new neighbourhood
- identifying paid supports and services to sustain your independence and lifestyle choices

People with physical disabilities, acquired brain injuries, mental health challenges, or anyone experiencing transitions in life (youth or older adults planning for their future)
