Service Description:

Memory care assessments and diagnoses, management services, and caregiver support services for people living with dementia, and their families
Memory Assessments - diagnostic assessment for people living with memory concerns, mild cognitive impairment (MCI), and other forms of dementia, medical history review, functional and driving safety assessment, physical assessment, personalized brain health information and lifestyle recommendations
Management Services - includes home safety assessments, medication optimization, and a medication reminder service
Seniors' Clinic - nurse practitioner-led full service primary care clinic for people age 65 and up
Care Partner Services - includes cognitive behavioural therapy for insomnia, "Dementia Doula" support service (includes caregiver coaching, healthcare system navigation, transitional expertise to retirement homes and long-term care settings), and personalized coaching
Professional Training - education and resources for healthcare professioanals in clinical, community, retirement home, and long-term care settings
