Community geriatric support * medicine, psychiatry, mental health outreach, intensive geriatric service workers, and memory clinics
Nurse Led Outreach Team (NLOT) supports long term care homes to reduce transfers to emergency department and shorten hospital stays
Behavioural Supports Ontario (BSO) clinicians assess and coordinate care for older adults exhibiting responsive and expressive behaviours associated with dementia, mental health, or addictions, and provide transitional and caregiver supports
