Service Description: Program that assists clients with chronic health conditions to develop and apply self-management skills to achieve their best health and wellness in partnership with health care teams and community support.

Workshops held at various locations in Guelph, Wellington County and Waterloo Region -- see workshop schedule for specific program locations

Programs include Take Charge, Take Charge - Chronic Pain, Craving Change, Moving Towards Change, Communication Skills, Peer Education Program (PEP)

**Workshops do not replace regular medical care or other treatment programs

Fees: None - free - participants receive course book and relaxation CD
