Service Description: Provides community care, emotional support, and practical assistance to people of all ages and their families facing a life-limiting illness, extending through bereavement.

Provides residential hospice palliative end of life care to clients with a life expectancy of three months or less.

Programs include support for palliative and bereavement clients:

Grief and Bereavement Support:
  • Adult Grief Support Group
  • Spousal Loss Support Group
  • Art Therapy Programs
  • Weekly Bereavement Walking Group
  • Volunteer Companionship Program
  • Grief and Bereavement Workshops

Palliative Support:
  • Weekly Palliative Day Program
  • Visiting Volunteers

Wellness Program:
  • massage
  • weekly yoga
  • lending library

Application: Residential hospice: referrals from health care providers and collaborative care partners * application form available on website for completion by Local Health Integration Network (LHIN) staff, Most Responsible Physician
