Service Description: Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes nurse practitioners, nurse specialists, pharmacists, dietitians, mental health therapist, social worker, clinical telemedicine nurses, health promoter, occupational therapist and physician assistant

Meetings: Take Charge! - 6 session workshop that helps people with any chronic or ongoing health conditions such as diabetes, heart disease, stroke, anxiety, pain, depression and arthritis to better manage their symptoms * meet other people who share similar challenges with their health and find new ways to deal with these challenges * caregivers welcome * sessions offered throughout the year, to register please call: 1-866-337-3318

Stepping Out Program - Free 6 week exercise program designed for people with chronic conditions including heart disease, diabetes, and respiratory disease * participants will exercise 2 times per week Tue 2pm-3pm and Thu 10am-11am at the Fergus Sportsplex * purpose is to introduce safe, effective and structured exercises, utilizing weight and cardio machines and increase participant's physical activity and confidence level

Cataract Trail Walking Group - Walk through nature on the Cataract Trail, Wed at 10am-11am meet us at the Forfar Park in Fergus located on Gartshore St

Dermatology Consult - One-on-one appointments, description and photo uploaded to specialist for assessment and consultation report * for more details, call Lucy at 519-843-3947 ext 126

Osteoporosis Workshop - Focuses on bone health, fracture prevention, medication and exercise to improve bone health, one day workshop held bi-monthly

Fibromyalgia Workshop - Focuses on education of the disease, pain management and role of exercise and stress management, 8 consecutive weeks, Fri 1pm-3:30pm

Back Care Workshop - Designed to assist those living with back pain, held bi-monthly on Thu from 9:30am-12noon

Cancer Support Group - A peer support group led by a cancer survivor, meets every 3rd Tue of every month, 10am-12noon

Diabetes Support Group - Open to anyone in the community who is managing diabetes * facilitated by a counsellor and a certified diabetes educator * 15 minute educational topic is discussed, followed by open discussion, meets every third Wed of the month from 4pm-5pm in Elora, 143 Metcalfe St

Healthy You: The Lifestyle Balance Program - Focuses on lifestyle changes to help patients reach and maintain a healthy body weight and prevent associated chronic diseases, provides education, support and results for weight management * delivered by dietitians and nurses * program is 1 year in duration consisting of 22 1-hour sessions, Sep 30, 2013 6:30pm-7:30pm in Elora, Oct 30, 2013 5pm-6pm in Elora, 143 Metcalfe St
