Service Description: Supports for people living with type 1 diabetes or gestational diabetes as well as for people with type 2 diabetes who are pregnant or are renal or cancer patients receiving care through WRHN * one-on-one or group education on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff

Programs include:
  • insulin pump therapy
  • education programs
  • insulin initiation



Fees: Free
Application: Call * medical referral required to participate in program * no referral required to participate in educational programs
Eligibility / Target Population: Adults 18 years and older with pre-diabetes, type 1 diabetes and gestational diabetes * adults with type 2 diabetes who are pregnant or are renal or cancer patients receiving care through WRHN
Languages: English
Language Notes: Provides translation services for several languages, including interpretation support for community members that are deaf or hard of hearing
