Service Description:
|
Supports for people living with type 1 diabetes or gestational diabetes as well as for people with type 2 diabetes who are pregnant or are renal or cancer patients receiving care through WRHN * one-on-one or group education on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
Programs include:
- insulin pump therapy
- education programs
- insulin initiation

Eligibility / Target Population:
|
Adults 18 years and older with pre-diabetes, type 1 diabetes and gestational diabetes * adults with type 2 diabetes who are pregnant or are renal or cancer patients receiving care through WRHN
