Self-Management Support Programs - Kitchener–Waterloo–Wellesley–Wilmot–Woolwich

Programs that focus on the support and empowerment of patients living with a chronic disease, and their families. These programs are designed to help individuals make realistic goals, build confidence in their ability in managing their health, and maintain an active and fulfilling life.
