Service Description: Refugees can connect with health care providers who deliver transitional health care and services, including primary care, specialist care and mental health supports * transitional care includes initial assessments, care and referrals to other health services * service is not for emergencies

Eligibility / Target Population: Refugees - newly arriving refugees and people arriving through other exceptional humanitarian authorizations * Resettlement Assistance Programs, sponsors, and settlement agencies

Language Notes: Translation support in over 200 languages is available upon request for the phone service only
