Service Description: Supports for people living with prediabetes or diabetes

One-on-one consultations on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * certified diabetes educators with registered nurses and registered dietitians on staff 



Fees: Most services are covered by OHIP
Application: Self-referral or by referral of a physician or nurse practitioner
Eligibility / Target Population: Adults 18 years and older with pre-diabetes, type 1 diabetes, type 2 diabetes, or gestational diabetes
